Other Policies

Cancellation and No Show Policy

As a general rule, if you need to cancel, we ask that you give us at least 24 hours notice.  This can give other patients more opportunities to be seen the same day or the next day.  Our office policy is that if you do not give us adequate notice for cancellation or you miss your appointment, you will be charged for the appointment.  If this occurs three times, you will be asked to find another provider.   If you miss your first appointment without reasonable notice, you will be asked to find another provider. 

Other Policies

We ask that you silence all phones and devices and do not use your cellphone in the office.  If you must take a call, please step outside so that you do not disturb other patients. 

 

If you have young children, it may be beneficial to secure childcare during your appointment.  This will help ensure that we can focus on your concerns and treatment.

New Patient Paperwork must be completed prior to the initial appointment.   You may bring in the completed copy or you may email it back to us prior to your appointment.   If we do not have a copy of the completed paperwork at the start of your first appointment, you will be asked to reschedule.
